Community Care Behavioral Health
is a psychiatric partial care day program licensed by the Division of Mental Health Services, led by a management team with over 25 years of professional experience working with consumers with severe and acute mental illness. The program provides a comprehensive array of psychiatric rehabilitation treatment and services that include case management, counseling, group therapy, skills building groups, co-occurring disorders groups, psycho education, psychiatric evaluations and medication monitoring, pre-vocational and work skills groups, therapeutic recreational and social activities. CCBH provides specialized tracks for individuals, who in addition to having a mental illness also have substance abuse issues or developmental disabilities.

Program Goals

1. TO PROMOTE WELLNESS & RECOVERY
by assisting consumers in identifying their valued community roles, maximizing their potential by reaching their personal, vocational, and educational goals, improving their re-integration into the community they reside in, and to reduce the risk of hospitalization.

2. TO EMPOWER EACH INDIVIDUAL
to develop necessary skills, improve the quality of their life and feel good about their self-worth.
